**For Abdominal Ultrasound: Your pet's abdomen (belly) will be shaved. 

In the event of an emergency, I understand that Loyal Family Veterinary Hospital will try to contact me as soon as possible. Please indicate below your wishes regarding CPR and other life-saving treatments (select one):

• Drop off appointments are scheduled in a time slot to allow time for each patient to get checked-in and reduce wait times. Please call the front desk if you have any questions regarding your drop off time. 
• Patient's dropped off for an exam - The doctor will call you after the exam to go over findings and recommended diagnostics or treatments.
• ALL patients for Specialist Appointments MUST be dropped off by 8AM 
• Specialist Deposits are $400 and must be paid at the time of scheduling to hold the appointment.
• We will call you when your pet is ready to go home.  Pick up is usually between 3-5:30pm.

CANCELLATION POLICY

Our goal is to provide quality medical care in a timely manner. No-shows, late shows and cancellations inconvenience those individuals who need access to urgent veterinary care. Therefore, we would like to make you aware of our policy regarding missed appointments and late cancellations.

You my be subject to a cancellation fee if you miss or cancel your appointment within 48 business hours of the scheduled appointment. 

Deposits are non-refundable if appointments are cancelled within 48 business hours of the scheduled appointment.
